Everything you need to know
Before Your Appointment
When preparing for your first appointment, bring the following with you when you come to the Shiley Eye Institute:
- A list of your current medications and dosages.
- The name, address and telephone number of the doctor who referred you, if applicable.
- Copies of your previous ophthalmic, surgical and medical records, if available
- If you have had imaging studies (CT scan or MRI), please bring the report and copy of the scan (typically available on a CD)
- Your insurance information card and Medicare card (if applicable).
- A picture ID (drivers license etc.).
Safety First
Updated Safety Initiatives to Protect Patients & Staff
The health and safety of our patients and staff is our highest priority. While some of these measures may pose an inconvenience, we appreciate your understanding and thank you for your assistance.
- The facility including exam rooms are cleaned & disinfected more frequently.
- Uponentry into the facility, each patient will be asked a series of health screening questions.
- For patient safety, physicians & healthcare personnel will be wearing masks, continually wash hands & social distancing when possible.
- We practice social distancing in our facility.
- There is a one-way path of travel going into / entering and out / exiting the building.
- We have removed magazines and shared items from the waiting areas in the lobbies.
- To accommodate social distancing in the lobby, please arrive no sooner than 15 minutes prior to appointment.
